offers daily in-home medical monitoring for patients with serious health
conditions to detect subtle changes in health status. Early detection of these
changes ensures immediate corrective action in the patient’s home.
The in-home wireless monitor, the size of a clock radio, provides VNA patients
the best of both worlds: the comfort of home and the security of daily
monitoring and medical attention.
The wireless monitor collects important health information, including:

Every morning the in-home monitor “talks” the patient through a user friendly
information collection process, then wirelessly transmits the date to the VNA
offices where advanced practice nurses review the information and takes
immediate action, where needed.
Since 2002, VNA TeleCare monitoring has successfully treated more than 500
patients. Daily monitoring of these patients’ health conditions has resulted in
improved health status and fewer hospital admissions and ER visits for these
patients.
